TERMS OF SERVICE

I acknowledge that I am voluntarily participating in services and activities provided by Atelier Sculpt, including but not limited to Pilates classes (reformer and mat), hot mat classes, red light therapy, Hydrafacial treatments, blowout and hair styling services, and any related wellness or beauty services (collectively, the “Services”).

ASSUMPTION OF RISK

I understand that participation in physical exercise, heat-based classes, aesthetic treatments, light-based therapies, and beauty services involves inherent risks, including but not limited to muscle strain, injury, burns, dehydration, dizziness, allergic reactions, skin irritation, adverse cosmetic results, and other unforeseen complications. I knowingly and voluntarily assume full responsibility for all risks, known and unknown, associated with my participation in the Services.

I acknowledge that hot mat classes involve elevated temperatures and that I am responsible for monitoring my own physical condition, hydration, and tolerance to heat. I understand that red light therapy, Hydrafacial treatments, and hair services may cause temporary or, in rare cases, adverse reactions, including skin sensitivity or irritation.

MEDICAL REPRESENTATIONS

I represent and warrant that:

• I am in good physical and mental health and have no medical condition that would prevent safe participation in the Services.
• I have consulted a physician if I have any concerns regarding exercise, heat exposure, skin treatments, light-based therapies, or cosmetic services.
• I will immediately inform instructors or staff of any discomfort, pain, dizziness, or adverse reaction.

I acknowledge that Atelier Sculpt  does not provide medical advice, diagnosis, or treatment, and that all Services are provided for general wellness and cosmetic purposes only.

RELEASE OF LIABILITY

To the fullest extent permitted by law, I hereby release, waive, discharge, and covenant not to sue Atelier Sculpt, its owners, employees, contractors, instructors, therapists, estheticians, stylists, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, loss, damage, or harm that may occur as a result of my participation in the Services, whether caused by negligence or otherwise.

INDEMNIFICATION

I agree to indemnify and hold harmless Atelier Sculpt from any claims, damages, costs, or expenses (including reasonable attorney’s fees) arising from my participation in the Services or my breach of this Agreement.

PERSONAL RESPONSIBILITY & CONDUCT

I agree to follow all studio rules, instructions, and safety guidelines. I understand that failure to do so may result in removal from a class or service without refund.

NO GUARANTEES

I understand that results from fitness, wellness, aesthetic, or beauty services vary by individual, and no guarantees—express or implied—have been made regarding outcomes.

CONSENT TO TOUCH

I acknowledge that certain Services may involve physical contact for instructional, therapeutic, or cosmetic purposes, and I consent to such contact.

PHOTOGRAPHY & MEDIA

I grant permission for Atelier Sculpt to use photographs or videos taken during my participation for marketing or promotional purposes unless I provide written notice otherwise.

SEVERABILITY & GOVERNING LAW

If any provision of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect. This Agreement shall be governed by the laws of the state in which Atelier Sculpt operates.
